Iowa Medical Record Fees: The 2025-2026 Statutory Guide

In Iowa, the amount a provider can charge for medical records is governed by the “Reasonable Cost” standard in Iowa Code § 622.10. While the statute does not list specific cents-per-page for all requesters, Iowa courts and providers generally default to the fee schedule set by the Iowa Division of Workers’ Compensation to define what is “reasonable.”

1. Statutory Fee Schedule (2025-2026)

The following rates represent the maximum allowable charges established under Iowa Admin. Code r. 876-8.9. These are the standard benchmarks used across the state in 2026 for both paper and electronic delivery.

The “Electronic Delivery” Cap

For records delivered electronically (via portal, email, or CD), most Iowa providers follow a $20.00 flat fee for the retrieval and transmission, regardless of page count. If a provider attempts to charge per-page rates for a digital file that exceed $20.00, it can often be successfully challenged as “unreasonable” under § 622.10(6).

2. The Patient Rate (HIPAA Protection)

Under federal law, which preempts state “reasonableness” if it is more restrictive, a patient requesting their own records is protected by the HIPAA cost-based limit.

3. Mandatory Free Records

Iowa does not have a broad state-mandated “free copy” statute for all disability claims like Illinois or Indiana. However, many Iowa hospitals have internal policies to waive fees for Social Security Disability (SSDI) requests if the request is sent directly from the Social Security Administration or a state-funded disability determination service.

When auditing an Iowa invoice:

  2. Identify Format: If the records were delivered via portal and the bill is $150.00+, dispute it. Under the standard of “actual cost of production” cited in Iowa case law, a digital transfer should not cost significantly more than the $20.00 retrieval flat fee.
